The Fine Print

 Participation Cost:

Standard membership is charged monthly at $269 and Premium membership is charged monthly at $320. Payments are processed in a 30-day billing cycle from the program’s start date. All members are obligated to participate in the membership program for at least 6 months.

Transition to Month-to-Month Billing

After the initial 6-month participation requirement is satisfied, the Participant’s obligation will shift to month-to-month with the same 30-day billing cycle.

Cancellation Policy:

Cancellation during the initial 6-month participation requirement:

Participants have the flexibility to cancel their participation within 10 days of the initial payment. Cancellation requests must be submitted in writing within that 10-day grace period. Requests for cancellation after 10 days will not be processed and participants will be required to pay for the whole membership contract of 6 months. There are no prorated refunds for cancellations or unused services.

After the 10-day grace period, clients are obligated to pay for the remainder of the 6-month minimum program. Even if a client decides they no longer want to use member services at Root & Branch, they will still be charged for the remainder of the membership program contract.

Cancellation during month-to-month obligation:

Cancellation requests must be submitted in writing at least 5 days before the start of the next billing cycle. Please note that any requests made within 5 days of the next billing cycle will not be processed in time to prevent payment and any charges already processed are non-refundable. There are no prorated refunds for cancellations or unused services. Following the date a membership is cancelled using the above 5-day guidance, the Participant is no longer entitled to any of the benefits of membership and will be required to pay for any services rendered after the date of cancellation.

Participants may also cancel their membership if they become physically unable to use Root & Branch services for sixty (60) or more consecutive days, and a Participant’s estate may cancel in the event of their death. Additionally, Participants have the right to cancel this membership if the clinic has closed permanently and is unable to deliver comparable services within 10 miles of the original location.

Unless otherwise specified in above, this Agreement is not cancelable by the Participant except as expressly provided above including, without limitation, by reason of Participant’s failure to utilize Root & Branch services.

No prorated fees or refunds will be offered for a Participant’s inability to schedule or utilize services during scheduled vacations or clinic closures that are part of regular clinic operations.

Root & Branch reserves the right to cancel a Participant’s membership and terminate this Agreement at any time for breach of the Rules and Regulations or generally undesirable behavior, as determined by Root & Branch in its sole discretion. Participants will not be entitled to a refund of any portion of fees or dues paid prior to the date of cancellation.

Regular Terms and Conditions

Members are subject to the same rules and regulations of regular clinic patients including but not limited to cancellation or rescheduling fees, appropriate conduct in the clinic space, and sufficient notice for scheduling or changes to the schedule. 

Scheduling and Availability

Root and Branch practitioners reserve the right to take time off for vacation or for illness where they may not be available for scheduling. Members are empowered to schedule with either Travis Kern, Travis Cunningham, or Olivia Rudio when setting appointments are Root and Branch regardless of which practitioner they usually work with. The inability of the patient to schedule with a practitioner does not nullify any terms of this contract.
